Information clause
regarding the processing of personal data,
for which the data controller is the President of the City of Zduńska Wola

Pursuant to Article 13(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), hereinafter referred to as RODO, we kindly inform you that:

  1. The Administrator of the personal data is the President of the City of Zduńska Wola based in Zduńska Wola at 12 Stefana Złotnickiego Street, tel: 43 825 02 00, fax: 43 825 02 02, e-mail: urzad_miasta@zdunskawola.pl
  2. The controller has appointed a Data Protection Officer whom you can contact by e-mail iod@zdunskawola.pl or by phone 43 825 02 82. The Data Protection Officer can be contacted in all matters concerning the processing of personal data by the Municipal Office of Zduńska Wola and the exercise of rights related to data processing.
  3. Providing personal data is a necessary condition for realisation of a case in the City Hall of Zduńska Wola. The general basis for data processing is Article 6(1)(b) and (c) and Article 9(2)(g) of the RODO and Article 6(1)(a) in cases where consent is given to provide contact details in the form of a telephone number and/or e-mail address.
  4. Personal data may be made available to other entities entitled to receive them under applicable laws, i.e. public administration bodies or entities acting on behalf of public administration bodies within the scope of applicable laws or other entities processing data on the basis of entrustment agreements, and furthermore to data recipients within the meaning of data protection regulations, i.e. entities providing postal services, IT services and others. Personal data will not be transferred to third countries on the basis of specific legal regulations, including international agreements.
  5. Personal data will be processed, including stored, in accordance with the provisions of the Act of 14 July 1983 on the national archival resource and archives (Journal of Laws of 2019, item 553 and 730). In the case of the processing of data on the basis of an expressed consent, for the period necessary for the implementation of the indicated purpose or to withdraw consent to the processing of personal data at any time without affecting the legality of the processing carried out on the basis of consent before its withdrawal.
  6. In relation to the processing of personal data, under the provisions of law, you have the right to:
    1) access to the content of your data, on the basis of art.15 RODO;
    2) to rectify your data, pursuant to Article 16 RODO;
    3) restrict processing pursuant to Article 18 RODO.
    In case of data processing on the basis of Article 6(1)(b), i.e. on the basis of a contract, you have the right to data portability.
    Furthermore, in the case of data processing based on your consent, you also have the right to:
    1) to have your data erased, pursuant to Article 17 RODO;
    2) object, pursuant to Article 21 RODO.
  7. In the case where data processing is based on your consent, you have the right to withdraw it, at any time, in the form in which it was given. The withdrawal of consent does not affect the lawfulness of processing carried out on the basis of consent before its withdrawal.
  8. You have the right to lodge a complaint to the supervisory authority – the President of the Office for Personal Data Protection, if you consider that the processing of personal data violates the provisions on personal data protection.
  9. When the provision of personal data is required by law, you are obliged to provide it. The consequence of failing to provide personal data will be failure to examine the case.
  10. The data will not be processed by automated means, including profiling.
